Adding a Medical, Dental, or Vision Plan and Loading Its Rates

Purpose

Before you can build a proposal for a client, the plans you want to offer must exist in UZIO with their rates. The Manage Plan page is where you add those plans, adjust an existing one (a different deductible, a custom Summary of Benefits and Coverage), and load the premium rates employees will see during enrollment. This article covers medical, dental, and vision plans. Supplemental, life, and disability plans have their own setup.

Who can do this: the client's broker, their agency, and account managers linked to that broker. See Getting Started on the Broker Portal for how those roles relate.

When to use this

  • A carrier's standard plan is missing or needs a change for one client.
  • You are offering the same custom plan to several clients and want to build it once.
  • Rates have changed for a new plan year and need loading before you build the renewal proposal.

You do not need to add a plan if the carrier's standard plan is already listed and correct. Standard plans are available to every broker on the platform without setup.

Choose the kind of plan first

When you open a benefit category, the Plan Category drop-down offers three kinds of plan:

Tip: To change one detail of a stock plan for a client, Copy the stock plan rather than building a new one. You keep every other detail and only edit what differs.

Add a medical plan

  1. Go to Clients, search for the client, and open the client's page.
  2. Click the Manage Plan tile under Core Operations.
  3. Click Medical, then Add Plan.
  4. Choose whether this is a shelf plan: Yes makes it available to all your clients; No keeps it to this client.
  5. Fill in the plan details (see the field reference below).
  6. Add promotional materials, such as the provider network, SBC, brochure, and formulary, as a link or an upload. This step is optional and can be done later.
  7. Enter the annual Deductible and Out-of-Pocket Maximum. Say whether the medical and prescription drug amounts are integrated. If either is not, enter the medical and drug amounts separately.
  8. For each plan benefit, enter a co-pay or a co-insurance amount.
  9. Click Submit, or Save & Add Rate to go straight to rates.

Plan details field reference

Note: If you enter both a co-pay and a co-insurance amount for the same benefit, employees see only the co-pay.

Dental and vision plans

A dental plan follows the same steps from the Dental category. The Out-of-Pocket Maximum is optional, because most dental plans do not define one. A vision plan is added from the Vision category; most vision fields are optional free text. Fill them in anyway: any field you leave empty shows as "NA" to employees.

Load the plan's rates

  1. From Save & Add Rate, or from Manage Rates on an existing plan, choose a rating strategy.
  2. Click Add Rates.
  3. Enter the rates for the strategy you chose, with their effective dates, and save.

Warning: Leave no gaps between age bands. If a spouse's or child's age falls in a band with no rate, the employee cannot choose coverage for them during enrollment. Do not overlap bands either.

Make the plan available

Use the Actions icons on the plan list to maintain a plan after you save it:

  • Preview shows the plan as an employee will see it. Use it to check that links such as the SBC and "See if your doctor is in network" work.
  • Activate is required for a custom or shelf plan before you can add it to a proposal. Stock plans need no activation.
  • Edit changes dates, plan configuration, and benefit details.
  • Copy clones any plan as a starting point.
  • Delete removes a plan that is not in use.

Common problems

Why can't I find my new plan when building the proposal? Custom and shelf plans must be activated first. Open Manage Plan, find the plan, and click Activate.

Why can't I delete a plan? Stock plans cannot be deleted, and neither can a plan that is part of a proposal. Remove it from the proposal first, or leave it in place if the proposal is already shared.

An employee says they can't add their spouse or child. Check the plan's age-banded rates. A missing band for the dependent's age blocks that coverage. Add the missing band in Manage Rates.

Employees see "NA" on a vision plan. Those fields were left empty. Edit the plan and fill them in.
